DINKLAGE MEDICAL CLINICS - WEST POINT
Dinklage Medical Clinics will be offering school physicals for $37 with a scheduled school physical appointment. Parents need to tell the clinic that it is for a school physical (college does not apply) when making the appointment. The clinics will accept insurance, co-pays, etc. Immunizations and/or other additional services will be at our normal charges.
Help provide the best possible physicals by:
1. Fill out the “History Form” to the best of your knowledge and bring it to the appointment.
2. Bring a first morning urine sample, if not possible - any sample is fine. To prevent unnecessary rechecks please have your child drink plenty of fluid 48 hours before the sample is taken. We can collect also at the appointment.
3. The student must bring their immunization records. This is an excellent time to get caught up on immunizations, especially for incoming 7th graders. Common immunizations include 4 Tetanus (includes pertussis for whooping cough), meningitis, and Gardisil for girls aged 9 and up. Goldenrod Hills Immunization Clinics are available also, call them at 800/445-2505 for information. Parents that won’t be at the appointment requiring immunization, need to contact the clinic and sign a permission form.
4. Girls are reminded not to wear a sports bra because of the scoliosis check.
Parents are encouraged to call or stop by any of the Dinklage Medical Clinics to speak with someone to clarify any information or to pick up any necessary forms.
At the end of each physical the student will be given a “Consent to Participate” and “Physical” form. Because of HIPAA laws, it is up to the parent whether additional information will be shared with the schools beyond the consent form.
MERCY MEDICAL CLINICS - WAYNE
Wisner Mercy Medical will be offering school physicals for $35 with a scheduled appointment.
Parents need to tell the clinic that it is for the $35 school special and no insurance will be submitted.
The clinic will accept insurance, co-pays, etc. Immunizations and/or additional services will be at our normal charges.
Make sure child has their school physical forms filled out and signed along with their immunization records for the appointment.
